Jaclyn Kehoe

Jaclyn joined National Care Advisors in 2024 as a Nurse Case Manager and has since transitioned to the business development team. She became a Registered Nurse in 2013 and previously worked as a Certified Nursing Assistant and Licensed Practical Nurse. With over 20 years of healthcare experience, Jaclyn has specialized in managing complex care needs, providing support to families experiencing homelessness, individuals with mental health and substance use disorders, those recently released from incarceration, and patients with frequent hospitalizations. She has also worked in benefits management and end-of-life care, holding certification as a Hospice and Palliative Nurse. Additionally, she is a Certified Case Manager with extensive knowledge of navigating resources for individuals with special needs, both professionally and personally.

Jaclyn holds a Bachelor of Science in Nursing from the University of Vermont and is based in Southern California. In her personal time, she enjoys spending quality time with her husband and three children.
