Sarah Kounanis

Sarah joined NCA in 2022 and brings more than 17 years of nursing experience, with strong expertise in case management and supporting individuals with complex medical, developmental, and elder care needs. Her nursing background spans medical-surgical care, outpatient surgery, home health, primary care, and end-of-life care, with a focus on care coordination and chronic disease management across diverse populations.

Sarah is a Registered Nurse, Certified Case Manager, and Public Health Nurse with a Bachelor of Science in Nursing. She is dedicated to enhancing quality of life for her clients through compassionate, individualized support and a collaborative approach to care. Outside of work, Sarah enjoys hiking, traveling, and spending time with her family.
